Discharge Planner

Brunswick Pointe is looking for an Discharge Planner to join our leadership team. The qualified candidate will be a self-starter, organized, and have strong customer service skills. Job responsibilities will include: discharge planning for post-acute skilled residents, MDS assessments, schedule and attend care conference meetings with residents and families, serve as a care advocate to promote positive customer service. Previous experience in nursing home/rehab setting preferred. Excellent computer skills including knowledge of Point Click Care and Microsoft Office preferred.
